Simple, from your phone
- Send your requestFill in the request form below — it takes about five minutes — or WhatsApp +91 80072 30250. You receive a reference number (for example SC-12345) straight away.
- Send your reportsAbove all, the written report from your own eye doctor, with your test results. Upload them with the form, or email them to services@surajeye.org with your reference number in the subject.
- We check your reportsOur team checks that the doctor has what is needed and asks you for anything missing. Incomplete reports are the most common cause of delay.
- The doctor repliesPlease allow 10 to 15 days. We confirm a time for your consultation or send a written opinion. For second opinions and new problems, we tell you the fee and how to pay first.
- ConsultSpeak with Dr. Sneha Rathod by WhatsApp video or voice, phone, or Google Meet or Zoom. She involves the right sub-specialist whenever needed.
- Advice in writingA written summary on WhatsApp or by email — in India with a prescription where appropriate; if you live abroad, advice to share with your local eye doctor.
What to send — and how
A safe opinion depends on good information. Photos of the eye help, but the doctor also needs the written report from the eye doctor who examined you.
📄 Please send
- The written report from your own eye doctor (ophthalmologist) — a letter, prescription or discharge summary with the diagnosis, vision, eye pressure and treatment advised. This is the single most useful document.
- Your eye test reports, with dates — for example OCT, visual field, fundus photographs, FFA, B-scan ultrasound, corneal topography or biometry — including the printed report, not only the pictures
- Operation or discharge notes, if you have had eye surgery or laser
- Your current spectacle prescription, and a list (or photo) of every eye drop and medicine you use
- Other health conditions, such as diabetes or high blood pressure
✅ How to send
- Easiest: upload them with the request form below
- Or by email to services@surajeye.org, with your reference number and the patient’s name in the subject
SC-12345 – Patient name – reports - Or on WhatsApp +91 80072 30250, quoting your reference number
- Send every page — as a PDF if you can. Photograph paper reports flat, in daylight, so that every word can be read
- Photos of the eye help only for the front of the eye (redness, swelling, a lump): daylight, no flash, in focus
- One email with all your reports is better than many separate messages

What a teleconsultation can — and cannot — do
We can
- Review your reports, scans and prescriptions
- Answer questions about your condition, treatment or surgery
- Adjust follow-up plans for existing patients
- Tell you how urgently you need to be seen
We cannot
- Measure eye pressure or examine the back of the eye
- Prescribe certain medicines without an in-person examination
- Manage emergencies — please call the doctor on call
- Replace a full eye examination when one is needed — we will arrange your visit
